In preparation for your first visit, please visit our Patient Forms page, and download your paperwork.
We suggest that you arrive 5-10 minutes before the start of your first scheduled appointment to complete our registration process.
Be sure you:
- Bring your ID
- Bring your insurance card or case number
- Bring your therapy prescription, if you have one
- Bring any relevant diagnostic reports you may have (for example, x-ray or MRI)
- Wear comfortable clothes and shoes
Also be prepared to answer questions such as:
- What types of symptoms are you experiencing?
- How long have you been experiencing these symptoms?
- How did it start?
- What aggravates and eases these symptoms?
Please tell your Physical Therapist about any previous Therapy Treatment you may have had, as well as the results.
Since physical therapy deals with the way your body moves, we’ll do a Physical Examination. We’ll take a look at your flexibility, strength and balance in the office. This can include having you walk, run and stretch in certain ways. It will help us pinpoint where you’re experiencing pain and where your movement is being inhibited.
After your Interview and Evaluation, your Physical Therapist will explain his or her assessment of your condition and explain the Individual Treatment Plan that has been designed for you.
If you have any questions regarding your Treatment Plan, be free to ask us anytime. Your understanding of Treatment Plan is essential to helping you achieve your goals, and is very important to a successful outcome.
Since you will only spend two to three hours a week receiving treatment at our office, you homework before and after treatment is very important. Execution your Home Exercise Program helps you attain your goals much faster.
Please remember to contact in with our Receptionist when you come out. She will coordinate follow-up treatment visits with you and your Therapist, and answer any questions you may have.
